got it finaly.

Mapname : ZS_3D_Anthem
Size : Small/Normal
Game Mode : Team-match(special Zombie Survival ed.) was in Deathmatch also nice guess Team Deathmatch also would work ;)
Waypoints : for Bravo only.

Looks are ok, and gameplay there could be nice. Btw, could you give to me IP and port of that survive server, I can't find it from the lobby.
