The picture got me at first glance, until I took the time to look at it closer.

The screenshot shows the Cel Shaded mod, then a bunch of stuff is drawn onto the screen, like that minimap. However, if you compare the minimap, it not only doesn't match the map being played, but the player positions it shows don't match where the players actually are by looking at the names on the side of the screen and extrapolating their relative position. As well, the minimap itself isn't drawn like a D3D minimap would be (due to how lines are drawn in D3D); it's rounded, which means it's either a vector drawing or done using the tools provided with Paint, Photoshop, etc., not done via a D3D injected hack.
