Microsoft Basic Optical Mousethis is the absolute choice for the pro-gamer.
Had it for a few years now. Very happy with it, and the thumb buttons are great as a push-to-talk button for voice chat programs. Also they are handy for navigating back and forward in web browsers.

Microsoft discontinued the sidewinder series with the actual sidewinders on them.
