This is more like amusing suicide but...

1. Get to the top of a buiding.
2. Wrap razor wire around your neck and tie to sturdy object.
3. Glue your hands to your head.
4. Jump off building.

Bonus points if you slam dunk your head into a basketball net.

Psychological torture?
Have someone completley hypnotised, while connected to life support systems or food/liquid injectors, for a long while (year?) - under the illusion of his sweetest fantasies such as being able to fly, having all the money in the world etc - then at a certain time wake him up from this illusion while he is strapped to a bed at a surgery room, completley paralysed due to chemicals, and start executing horrible torture ways such as cutting out non fatal veins, chemical experiments with the eye tissues, peeling off the skin, and basically anything your heart desires, untill he passed out due to extreme pain. Finally, cutting his head open so you could pour acid over his brain would be a good idea, ultimately killing him, making it the worst possible death.. psychological and physical.

That, should pretty much be a b***h.

The acid over the brain is a nice touch - but might lead to death too quickly. I suggest (as STM has already I believe) immersing other parts of the body in acid, preferably after skinning and then cauterizing them with a blow-torch or hot coals.

Nevermind what you do to him really, all that matters is that he goes through the ultimate worst death psychologicaly and physicaly, by making sure of two things:

1. Psychological effect: to drift the man away from his dreamworld where he was a god to himself, had anything that he desired and constantly enjoyed the best feeling his mind can construct -- onto the horrible reality where he is being held paralysed, helpless and extremely fragile.
2. Physical effect: to hurt the man in any desired way, to the point when he can no longer take it and passes out, (a usage of some chemicals that will keep him conscious longer might be a good idea for even more pain) only then kill him, nevermind how - he already passed out of pain.

The most painful way? I'd hate to be waterboarded with vinegar until the vinegar burns through my skin. I'd be kept alive by IV. After every wash of vinegar I'd have salt poured over my body. Once my skin is nice and ripe it would really suck if half of me was put into an icy bath and the other over a fire. Then switched every now and then. Not enough hot and cold to do any real damage, just enough to hurt. Once I'm ready to pass out from pain it would be just awful if I was given stimulants to keep me awake, and possibly even accelerating my senses and making everything hurt more. Once the vinegar is through my skin and I'm starting to bleed it would be horrible if my entire body was wrapped in a cloth with little needles pointing inwards, that slowly rotate around in lazy circles. Then I would be repeatedly rolled around so that parts of the needles dig in farther and make tiny rips all over me. After this the cloth and needles would be ripped off of me. As I die I would be kicked and punched repeatedly by those who I love in the crotch and everywhere else. Then I would be thrown into the ocean to drown with the saltwater.

Yeah that would really suck.

I always thought the worst way to die would be being squeezed to death over a longer period, say, a day. Imagine being strapped to a concrete wall, whith another wall so close you have to turn your head to the side to fit. Then slowly gaining in on you, 1,5 milimeter an hour.
