default map when no maplist is ctf_Death

First, please change the "default" map to something else than ctf_Death. If the server has no maplist, than it will start with ctf_Death, which isn't a default map anymore.
This can be solved, of course by setting a maplist, which is highly recommended, but it's a bug anyhow.

Also, when there is no maplist, on map change the server stops responding to REFRESHX

So if these could be fixed, it would be sweeeet.  ;D
