Help Filling Out Blanks for Port Forwarding

Can someone help me fill this out? I've read the sticky, it's just the blanks are a little different from the directions..

Thanks Enesce, but I think I did soemthing wrong because it isnt working, a little more help please? This is what I have down right now

When I connect with Soldat Admin[which can connect to the server] it has the same thing, and it keeps going until tons of popups come up from Soldat admin telling me about a violation or something

Delete the numbers after the "Soldat" string in the first inputs.

eh? sorry, I'm not very good at this stuff

Replace "Soldat1" with "Soldat", "Soldat2" with "Soldat", and so on...

Turn off super-fantastic-uber scripting and those access violations wont popup
